Quality Improvement: Continuous Quality Improvement (CQI) Plan
A complete Performance Improvement Plan will be developed that will guide your quality improvement activities with the goal of providing the best care possible, based on community standards defined for Radiation Oncology. “Best Practice” has an inherent component of measurement. Any “Best Practice” system must be able to gauge its own performance against independent and objective measurements, thereby learning from the past practices and ensuring that community standards of care are met. These measurements can then be used across organizations for the purposes of benchmarking.
A team-planning meeting will be conducted in order to establish the ‘performance measurements’ that will be monitored in your department during the upcoming year. Performance Measurements monitor areas of high risk, high volume, or problem prone areas. Benchmarking and collecting data on identified activities will ensure that quality care is provided and that areas that can be improved are identified. Performance Measurement activities will be used to collect bench-marking data for new centers.
Key processes are defined as elements of service that required 100% of the time for 100% of the patients to ensure quality patient care. Key processes will be the elements that are checked in chart rounds, ensuring compliance that all community standards are performed in your department for all your patients. This program outline will include chart rounds, peer review and any other standard meetings that are conducted in your organization.
The interviews and customized CQI program will include the following:
- Defining Key Processes, Performance Measurements and Outcome Studies.
- Developing the appropriate methodology for data-collection and documenting all program data.
- Determining processes for assessing outcome data.
