Churchill Consulting, a Division of CruxQS: Notice Regarding Client Privacy
Maintaining the trust and confidence of our clients is a high priority. That is why we want you to understand how we protect your privacy when we collect and use information for you, and the steps that we take to safeguard that information. This notice is provided to you on behalf of Churchill Consulting.
Agreements we require:
Prior to providing any services, that will result in obtaining or having access to confidential information, Churchill Consulting will enter into a contractual agreement with Client. Churchill Consulting includes a contractual provision that a HIPAA Business Associate Agreement be provided by Client to Churchill Consulting prior to providing any services.
Information we collect:
In connection to providing services, we obtain non-public information and protected health information about you and your patients including:
- Protected health information or “PHI”, which includes patient information from current medical records
- Organization billing information, such as fee schedules
- Information about organization’s coding and billing practices
- Information in regard to organizational policies and procedures
Non-disclosure of information:
Churchill Consulting will not disclose any information at any time regarding you or your patients under any circumstances, except as allowed and stipulated by contract for agreed upon work to be rendered, as specified by client. All information supplied to Churchill Consulting is used solely for the purpose of specified contract work and is not used for any other purposes.
Our security policy:
Only those individuals who need it to perform their jobs are authorized to have access to confidential client information. We maintain physical, electronic, and procedural security measures that comply with applicable state and federal regulations to safeguard confidential client information. We have implemented administrative, physical, and technical safeguards that reasonably and appropriately protect the confidentiality, integrity, and availability of the electronic PHI that may be created, that has been received, which is maintained, or may be transmitted on behalf of Client, as required by the HIPAA Security Rule.
Completion of services:
At termination of contract or upon completion of service, Churchill Consulting will, if feasible, return or destroy all PHI received from Client. Churchill Consulting will retain no copies of any PHI, or, if such return or destruction is not feasible, extend the protections of the contract to the PHI and limit further uses and disclosures to those purposes that make the return or destruction of the PHI infeasible. All paper versions of any PHI, if not returned to Client, will be shredded within 30 days of completion of contract services.
Closed or inactive clients:
When we complete contract services or you become an inactive customer, we will adhere to the privacy policies and practices as described in this notice.
